Your assessor will review your initial skills, literacy and numeracy checks, confirm suitability and agree support before enrolment is finalised. Submitting this form or uploading ID does not confirm registration. Qualification assessment must not start until your registration with ProQual is confirmed.

Assessment, induction and your own work

Your assessor will explain the qualification requirements, learning outcomes, assessment criteria, evidence requirements and assessment methods, then agree an assessment plan with you. ProQual qualifications are assessed and certificated in English. The initial checks above help identify support needs; they do not replace the assessor’s English-language and literacy review, skills scan, identity-verification statement or informed-induction record.

Submit your own work and acknowledge sources, including websites and any permitted use of AI. Follow the centre’s assessment and malpractice rules. Ask your assessor before using AI or outside assistance for assessment. Proven malpractice can affect assessment results and certification. An authenticity declaration is also needed when you submit assessment evidence.

Reasonable adjustments and special consideration

Tell us as early as possible if you need help with access, reading, writing, communication, technology or another aspect of learning or assessment. Use the support-discussion option above or contact the centre privately. Please do not put medical reports in this public-facing application form.

Reasonable adjustments are considered individually and agreed before assessment. They may change the way you access or complete an assessment while keeping the required competence standards unchanged. The centre must record the request, supporting evidence, decision and arrangements. A support request on this form is not approval.

Special consideration concerns temporary illness, injury or adverse circumstances around an assessment. Contact your assessor immediately if affected. The centre must obtain your express consent and seek ProQual approval before implementing special consideration; it cannot waive unmet competence requirements. You may appeal a decision using the centre procedure.

Complaints, appeals and certification

Raise concerns first with your assessor or contact info@londonfireconsultants.co.uk. Ask for the centre’s complaints or appeals procedure and applicable deadlines. For an assessment decision, request an explanation and follow the centre appeal route. You must exhaust the relevant centre procedure before escalating an eligible case to ProQual. ProQual’s procedures have short deadlines, so raise concerns promptly. Contact enquiries@proqualab.com for guidance.

Certificates are claimed only after successful achievement, the necessary assessment and quality-assurance checks, and authorised centre confirmation. Check your full name carefully. Completing this application is not a qualification award or certification decision.

Privacy and data protection

Application and training records: We collect the identity, contact, course, experience and initial-check information you provide to consider your application and administer your training, assessment and certification. Core application processing is necessary for steps you request before a training contract and for delivery of the agreed service (UK GDPR Article 6(1)(b)). Qualification administration, identity assurance and quality checks also support our legitimate interests in operating an accountable training centre and preventing misuse (Article 6(1)(f)). Required fields support application review and registration; without the necessary details we may be unable to progress your application. ID upload is optional where an alternative identity check is arranged. Staff review your application and initial checks; this form makes no automated admission or qualification decision.

Sharing and storage: This website uses Squarespace to receive and store submissions, with enrolment notifications sent to the centre. Relevant personal data may be transferred to ProQual for registration, certificates, assessment and quality assurance, investigations, reasonable adjustments and special consideration. ProQual may contact you directly, including by personal telephone, and disclose relevant information to regulators or sector bodies where required. Read ProQual’s privacy notice for its processing arrangements.

Retention: ProQual requires relevant candidate, programme and assessment records to be retained for at least three years after certification. Different periods apply to particular records, including adjustment/special-consideration logs and investigations. ID copies are deleted within 30 days after ProQual confirms a successful qualification claim, or within 30 days of withdrawal from the course. The identity-verification record is retained with the core learner file. Unsuccessful or withdrawn applications are deleted 6 months after the final decision or application withdrawal. If an application does not proceed to enrolment, any ID copy is deleted within 30 days of closure. An active complaint, dispute or investigation may require relevant records to be kept longer; the reason and review date are recorded.

Your choices and rights: You can request access, correction, deletion or restriction, object where applicable, request data portability where it applies, and withdraw consent for optional processing. Withdrawal does not affect processing already lawfully carried out. Some records must be retained for qualification, legal or regulatory reasons. Contact the centre about your rights or raise a data-protection concern with the Information Commissioner’s Office. Contact the Centre Manager at the email address above to exercise a right or raise a complaint.

Access: The Centre Manager and designated enrolment administrator handle identity records. Assigned assessors and IQAs receive only the information needed for their work. A designated equality-monitoring lead handles identifiable monitoring responses; routine reports use anonymous totals. Access to submission accounts and notification mailboxes is restricted to authorised staff carrying out these roles.

Service providers and international transfers: Squarespace and its service providers may process form data outside the UK, including in the USA. Squarespace describes its UK Extension to the EU-US Data Privacy Framework and fallback contractual safeguards in its Data Processing Addendum. Contact the centre for information about the safeguards applying to your data. Google reCAPTCHA is used to protect these forms from spam; see Google’s privacy policy.

Initial-check sources

The literacy and numeracy scenarios use the Regulatory Reform (Fire Safety) Order 2005: Article 9 — risk assessment, Article 14 — emergency routes and exits and Article 21 — training. The figures in the numeracy questions are fictional examples; they are not legal limits or required training durations.

Voluntary equality monitoring

The separate form below helps us review equality of access, participation and outcomes. You may skip it entirely, leave a question unanswered or select Prefer not to say. These answers are not part of your skills assessment. They must not be used to make discriminatory entry or assessment decisions.

We rely on your consent (Article 6(1)(a)) and explicit consent for ethnicity, disability / health and learning-difficulty information (Article 9(2)(a)) to use the monitoring answers you choose to provide. Relevant monitoring information may be shared with ProQual for the same equality-monitoring purpose. Consent is separate from the enrolment declaration; declining does not affect your application, support or assessment. If you provide a centre learner reference, authorised staff can link the monitoring record to participation and achievement information. Without a reference, it supports general monitoring only. Do not enter your name, email, date of birth, ID documents or medical details in this form.

You can withdraw consent by contacting the centre with your reference and approximate submission date. A response that cannot be identified cannot be individually retrieved or withdrawn; published anonymous statistics cannot be reversed. Identifiable monitoring responses are retained through your course and for 12 months after completion or withdrawal, then anonymised for statistics and the identifiable responses deleted. A response that cannot be linked to a course is anonymised or deleted within 12 months of submission. Identifiable data is reviewed sooner if you withdraw consent.
